Members under the age of 13 must have a parent/guardian accompany them in the facility and a liability form must be signed by their parent/guardian. Members aged 13-18 do not need parent/guardian supervision; however, their parent/guardian must sign the liability form. 

No! There are no additional fees outside what is listed.
